Natural Healing Chiropractic LLC, doing business as Mycelium Chiropractic & Neurology ("we"), operates Mycelium CRM, an internal application that connects with Meta platforms (Facebook, Instagram and Meta advertising tools) to manage our own pages, ads and patient inquiries. This policy explains what information we access, how we use it and the choices you have.
1. Information we collect
- Lead forms: information you voluntarily submit through our Facebook or Instagram ads, such as name, email, phone number and your answers.
- Messages and interactions: messages, comments and other interactions you send to our Facebook Page or Instagram account.
- Ad and page data: aggregated performance data for our ads, Page and Instagram account (reach, impressions, clicks, spend). This data does not identify individuals.
- Login data: when our authorized staff log in with Facebook Login for Business, we receive basic profile information and the tokens needed to operate the application.
2. How we use information
- To respond to your inquiries, schedule appointments and follow up on your requests.
- To manage our Facebook Page, Instagram account and advertising campaigns.
- To measure and improve the performance of our ads.
- We do not sell, rent or trade your personal information, nor do we use Meta data for any other purpose.
3. Disclosure
We only share information with service providers that help us operate the practice (for example, hosting or email), who are obligated to protect it, or when required by law. Meta data is handled in accordance with Meta's Platform Terms and Developer Policies.
4. Health information
Mycelium CRM is used for marketing and communication purposes only. Please do not send detailed medical information through Facebook or Instagram. Clinical records are kept separately and are protected by applicable health privacy laws.
5. Retention and security
We retain personal information only as long as necessary for the purposes described or as required by law, and we apply reasonable administrative, technical and physical safeguards to protect it.
6. Your rights and data deletion
You may request access to, correction of or deletion of your information at any time by emailing myceliumchiropractic@gmail.com with the subject "Data Deletion Request". Include the name, email or phone number you used to contact us. We will confirm and complete your request within 30 days. You can also remove the app's access from your Facebook settings (Settings & privacy › Settings › Business integrations / Apps and websites).
7. Changes to this policy
We may update this policy from time to time. The effective date indicates the latest revision.
